4. Complete & Follow-Up
Work is completed, tested, and documented for your records.

Your have two options to submit your WORK ORDER or REQUEST SERVICE. #1 Complete the form below or #2 You can call 262-300-5339 and when prompted for the reason for your call simply say “I want to submit a work order or Request for Service” We will ask you all the questions needed to submit a work order or service request.
